All students and volunteers participating in activities hosted by CentreSTEM, including Centre County 4-H Robotics programs in a CentreSTEM facility, need to complete the waiver linked below prior to the activity. To complete the waiver:
Who should complete the waiver?
If the participant is age 18 or older at the time of the activity, he or she should fill out and e-sign the waiver themselves.
If the participant is younger than 18 years old at the time of the activity, a parent or guardian should fill out and sign the waiver on behalf of the participant.
If multiple people from your family are participating, please fill out the waiver once for each.
How do I complete the waiver?
Click on the link below to access the online waiver securely through Adobe.
Read the waiver completely.
Enter the name of the Participant (see above) in the appropriate box.
Complete the e-signature.
When you submit the document, it will request your email address. Enter a valid email address for the signer when prompted.
Adobe will email you a confirmation. Follow the link to verify your email address.
You will then be emailed a copy of your signed waiver. CentreSTEM will also receive the signed waiver, so you do not need to forward it to us.
